Overview

Introduction

Topamed 50mg Tablet is a medicine used to treat epilepsy (seizures) and to prevent migraines. It is also used in the treatment of Lennox-Gastaut syndrome (a rare, but severe form of epilepsy that starts in early childhood).

Topamed 50mg Tablet controls seizures by decreasing the abnormal and excessive activity of the nerve cells in the brain. It may be taken with or without food, preferably at the same time, each day. If you have missed a dose, take it as soon as you remember it and finish the full course of treatment even if you feel better. Stopping the medication suddenly without talking to your doctor may cause increased frequency of seizures. Your doctor may advise regular monitoring of serum bicarbonate levels while you are taking this medicine.

Some of the common side effects of this medicine include nausea, taste change, fatigue, diarrhea, weight loss, loss of appetite, and memory impairment. It may also cause vision problems like myopia or glaucoma, so it is important to get your eyesight checked before you start the treatment and regularly thereafter. You may experience dizziness and sleepiness, so do not drive or do anything that requires mental focus until you know how this medicine affects you. Please consult your doctor if these side effects bother you or do not go away.

How to cope with side effects?

The occurrence of side effects varies from person to person. The following are a few ways of dealing with some of the common side effects. However, consult your doctor if these persist.

  • Coping with Abdominal pain
    In most cases, this is mild enough to be treated at home. Try to rest and relax. Putting a heat pad or covered hot water bottle on your stomach may also help. Having a warm bath or shower can help you relax and relieve the pain. Drink plenty of clear fluids such as water and reduce your intake of coffee, tea, lemon juice or soda. Avoid alcohol and smoking as these can aggravate the pain. Eating slowly and having smaller and more frequent meals can also help. If the pain is severe, speak to your doctor. They may be able to guide you better.
  • Coping with Diarrhea
    Keep up your intake of fluids and electrolytes (sugars and salts) to avoid getting dehydrated. Eat less fiber (avoid raw fruits, fruit juice and vegetables). Talk to your doctor about possible medication to manage diarrhea. Ask about reducing the dosage of your drug or other suitable treatments.
  • Coping with Dizziness
    This is usually short-lived and should go away within a few days. If this happens, stop what you are doing and sit or lie down until you feel better. Lying still in a dark, quiet room may help reduce the spinning feeling. Sleep with your head slightly raised on two or more pillows. Get up slowly from a lying or sitting position. Get plenty of rest and try to relax as being anxious can make it worse. Try taking this medicine at bedtime to reduce the symptoms. Drinking plenty of water and ginger tea may also help. Avoid alcohol, caffeine and smoking as it will make you feel worse. Avoid driving or using tools or machinery until you feel better.
  • Coping with Loss of appetite
    It is important to eat, even if you do not feel like it. You might try switching to smaller meals and frequent snacks. Try eating healthy and nutritious snacks that are high in calories and protein, such as dried fruit and nuts. It is important to drink even if you cannot eat. But, do not fill your stomach with liquid before eating. Eat when you are most hungry. Exercise such as a short walk may also increase your appetite. Talk to your doctor about possible medication or supplements if your condition does not improve.
  • Coping with Memory impairment
    There is a lot you can do to improve your memory. Keep active and take regular exercise to improve blood flow to your whole body, including your brain. Eat a healthy diet so your brain gets the nutrients it needs. Some foods containing omega-3 fatty acids (such as oily fish) may benefit brain health. Keep mentally active. It is important to socialize, so stay in contact with friends and family. Stay organised – keep a diary for things you have to do, keep things you need regularly (such as keys, purse and spectacles) in a set place and do not try to do too many things at once. Make sure you get good sleep, make it a priority. Drinking alcohol can lead to confusion and memory loss, so try to cut down or stop. Also, stop smoking because it reduces the amount of oxygen that gets to your brain. If you are worried about memory loss, talk to your doctor, especially if it affects your ability to live your daily life.
  • Coping with Nausea
    You can help yourself by eating small, frequent meals rather than large ones and drinking plenty of fluids. Eat slowly. Avoid fatty, fried, spicy and very sweet foods. Eat cold or slightly warm food if the smell of cooked or cooking food makes you feel sick. Get plenty of fresh air. You could also try chewing ginger or drinking ginger tea. Eat bananas to replace potassium in your blood which can drop if you are sick (vomit). Use oral rehydration salts to replace vitamins and minerals lost through being sick. There are some medicines that can help you stop from feeling sick. Speak to your doctor if your condition does not improve.
  • Coping with Nervousness
    This often disappears when the condition you are being treated for is managed. You could ask your doctor about changing the dose of your medicine or alternatives if it is affecting your daily life or causing you distress. Psychological treatments may be available. Some things you can do to help yourself include stopping smoking, cutting down the amount of alcohol and caffeine you drink, and doing regular exercise. You should also eat a healthy diet and find out how to get sleep if you are having trouble. Try talking to a friend or family member. Calming breathing exercises and other relaxation techniques may also help.
  • Coping with Numbness
    Massaging the numb area can help improve blood flow and provide relief. Try warming your hands and feet if they are cold. In case they are hot, try cooling them down. Get plenty of rest as lack of proper sleep may worsen the condition. Reducing stress, avoiding alcohol, exercising regularly, and getting enough vitamins and nutrients through your diet can also make you feel better.
  • Coping with Sleepiness
    If the medicine is making you drowsy during the day, stop what you are doing and sit or lie down until you feel better. You can also consider taking a nap. Physical exercises such as walking may be helpful. Do not drink alcohol, as it will make you feel more tired. Avoid driving or operating heavy equipment when you are feeling drowsy. This problem usually goes away as your body gets used to the medicine. However, if it does not, ask your doctor if you can take your medicine at bedtime or whether the dose can be reduced.
  • Coping with Taste change
    Keep your mouth clean. Brush your teeth and rinse your mouth with saltwater or mouthwash after each snack or meal. Use plastic or glass utensils if the food tastes like metal. Avoid very hot or very cold foods. Chewing mints or gums may also help in improving the taste. Increase your fluid intake and choose foods that have strong flavors. Try adding garlic, lemon juice, herbs, spices and pickles or chutneys. Avoid cigarette smoking.
  • Coping with Weight loss
    As a general rule, speak to your doctor if you lose weight while using this medicine. Losing too much weight can lead to malnutrition. Increase the amount of food you eat. Try eating foods that are high in calories and protein. Drink warm milk instead of tea every so often. Eat smaller meals and eat more often. There may be medicines and physical activities that can improve your condition.Speak to your doctor about this.

How to use Topamed Tablet

Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Topamed 50mg Tablet may be taken with or without food, but it is better to take it at a fixed time.

What if you forget to take Topamed Tablet?

If you miss a dose of Topamed 50mg Tablet, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.

Epilepsy requires regular treatment with anticonvulsants like encorate chrono for at least 3 years of seizure free period after which medicines may be tapered off unless seizures reappear. At present, there are no other effective treatment options.

Q. For how long do I need to take Topamed?

The dose and duration of this medicine will depend on your doctor’s advice. Topamed does not cure your condition (seizures/migraine) but prevents them from occurring. So, you may have to keep taking it for months or years depending on improvement in your symptoms and side effects that may occur.

Q. Is Topamed good for anxiety?

Topamed is not indicated for the treatment of anxiety. Please consult your doctor before taking the medicine.

Q. What are the serious side effects of Topamed?

Serious side effects of Topamed are rare. These serious effects of Topamed may include glaucoma (difficulty in seeing, eye pain and blurred vision), kidney stones (sharp pain in your sides or lower back) and metabolic acidosis (tiredness, loss of appetite, irregular and faint heartbeat). Other serious side effects of Topamed could be suicidal thoughts and insufficient sweating (some children taking Topamed may not sweat enough in hot weather, causing their body temperatures to rise).

Q. I am taking valproic acid for fits. Will Topamed interfere with the working of valproic acid? Can this combination be taken?

Topamed does not interfere with the working of valproic acid, but it increases the risk of getting increased levels of ammonia in blood (hyperammonemia). This can be dangerous. Excess levels of ammonia in blood may cause difficulty in thinking, remembering information, or solving problems. It may decrease your alertness and make you feel very sleepy with low energy. Tell your doctor immediately if you notice these symptoms. In rare cases, hyperammonemia may even occur because of Topamed alone.

Q. Can Topamed be used in children?

Yes, Topamed is used for seizures in children more than 2 years of age. However, because of the lack of any reports, this medicine is not recommended to be used in children for migraine.

Q. Since I have started using Topamed people are saying that I have lost weight. Is it because of Topamed or something else for which I should get investigated?

It could be due to Topamed because Topamed commonly causes weight loss probably due to decrease in appetite. If you still have any doubts, talk to your doctor who will advise you appropriately.
