So, when used with blood pressure lowering drugs or nitrates, it can cause a severe fall in blood pressure, which could be fatal. Sildenafil should not be taken if you are taking nitrates for chest pain or you have a history of heart attack or stroke in the past six months.
    Q. What are sildenafil citrate sublingual tablets?
    Sildenafil sublingual tablets start to act within 15 minutes with lesser dose compared to oral tablets which take almost 30 minutes to 1 hour to achieve the same effect. These are not taken with water. Sublingual tablets dissolve under the tongue, get absorbed and directly reaches the blood stream and act much faster compared to the oral tablets. These are developed to avoid the breakdown of the medicine in stomach and liver before reaching the blood circulation.
    Q. Is sildenafil safe to use in patients with diabetes?
    Yes, sildenafil is safe to use in patients with diabetes. Diabetes is one most common causes of erectile dysfunction and is commonly reported by these patients. Long standing diabetes mellitus can damage the nerves in the body (neuropathy) including the nerves of the penis. It also harden and narrow the blood vessels which decreases the blood flow to the penis causing erectile dysfunction. Sildenafil is seen to be beneficial in treating erectile dysfunction (ED) in patients with diabetes mellitus.
    Q. Why is the use of sildenafil contraindicated with nitrates?
    Use of sildenafil is not recommended in patients taking nitrates (organic nitrates or organic nitrites) as their combined use can cause a severe fall in blood pressure, which can also result in death. It is advisable to keep a time gap of 24 hours between the intake of sildenafil and any nitrate substance.

    Sildenafil is indicated for use in women for the treatment of pulmonary arterial hypertension (PAH). However, it is not recommended in women of childbearing age group.
    Q. What are the serious side effects of sildenafil?
    Sildenafil can cause serious side effects like heart attack, sudden cardiac death, abnormal heart beat and bleeding in the brain tissues that can cause temporary or permanent brain damage. A patient can have chest pain during the intercourse and if this occurs, try to relax in a semi-sitting position and do not use any nitrate to relieve the chest pain.     Both sildenafil and lodenafil are phosphodiesterase-5 (PDE5) inhibitors and are used for the treatment of erectile dysfunction.     No, sildenafil is not indicated for the treatment of piles (hemorrhoids). Rather, its use has been reported to cause piles (hemorrhoids).

    Sildenafil undergoes rapid absorption in the intestine. However, only 40% of the drug reaches the blood in its active form because of its breakdown in the gut wall and liver before reaching there.

    Sildenafil can be given by intravenous (i.v.) route in the management of pulmonary arterial hypertension (PAH) and not in the treatment of erectile dysfunction (ED). The injectable form is used in patients already taking oral sildenafil or those patients who are unable to take the drug orally.
