Brain fog can make simple tasks feel strangely difficult. You may read the same sentence twice, forget why you opened your phone, or struggle to find the right word during a conversation. Poor sleep, stress, dehydration, long workdays, and several health conditions can all contribute to that cloudy feeling.

So where does creatine fit in?

You probably know creatine as a popular supplement for muscle strength and exercise performance. But researchers have also been studying whether it can support the brain. Some recent studies have found benefits for memory and certain aspects of mental performance. At the same time, other research has found little or no effect.

That makes the real question less exciting but more useful: can creatine actually help with brain fog?

What is Brain Fog, Exactly?

“Brain fog” isn’t a medical diagnosis. It’s a common way of describing problems such as poor concentration, forgetfulness, mental fatigue, and difficulty thinking clearly.

You might notice it after several nights of poor sleep or during a stressful period. It can also occur with some illnesses, nutritional deficiencies, hormonal changes, and medications.

This issue matters because brain fog doesn’t have one single cause. Taking a supplement without addressing the reason behind it may not solve the problem.

Creatine does not treat brain fog. The research is looking at specific areas of cognitive performance, such as memory, attention, and processing speed.

Why Would Creatine Affect the Brain?

Your brain uses a lot of energy. Creatine is involved in the body’s system for storing and quickly supplying energy to cells, including brain cells.

Your body makes creatine naturally, and you also get it from foods such as meat and seafood. Creatine supplements can increase creatine stores in the body, although getting creatine into the brain is more complicated than getting it into muscle.

Researchers think extra creatine may help the brain maintain its energy supply when energy demands are high. This could be particularly relevant during periods of sleep deprivation or mental stress.

But there’s an important catch. A biological reason for something to work doesn’t automatically mean you’ll notice a real-world benefit.

What Does the Latest Research Say?

One of the most useful pieces of evidence is a 2024 systematic review and meta-analysis that looked at 16 randomized controlled trials involving 492 adults [1].

The researchers found that creatine supplementation was linked with improvements in memory, attention span, and information processing speed. However, creatine didn’t significantly improve overall cognitive function or executive function. The researchers also rated the certainty of evidence differently across outcomes, with moderate certainty for memory but lower certainty for several other measures.

That sounds promising, but don’t read it as “creatine cures brain fog.”

The studies measured specific cognitive tasks. Brain fog is a much broader experience. Someone may perform better on a memory test without suddenly feeling completely clear-headed throughout the day.

Another systematic review published in 2024 reached a more cautious conclusion. It found that creatine can increase creatine levels in the brain, but the evidence for an actual improvement in cognition remained mixed. The researchers also pointed out that study designs and dosing methods varied, making firm conclusions difficult [2].

So, yes, there’s a signal. But it’s not a slam dunk.

Creatine May be More Useful When You’re Sleep-deprived

This is one of the more interesting areas of research.

In a controlled study, researchers gave participants a single high dose of creatine during a period of around 21 hours of sleep deprivation. The study found improvements in cognitive performance along with changes in brain energy-related compounds.

That doesn’t mean you should use creatine to replace sleep.

If you’re sleeping four or five hours every night and hoping creatine will rescue your concentration, you’re asking a supplement to do a job that sleep does much better.

Still, the findings suggest that creatine could have a role when the brain is under increased energy stress. More research is needed to understand who might benefit most.

What About Memory?

Memory is probably where the evidence looks most encouraging.

An earlier systematic review and meta-analysis of healthy individuals found that creatine supplementation improved memory compared with placebo. The benefit appeared stronger in older adults than in younger participants.

The newer 2024 meta-analysis also found a significant improvement in memory [1].

This doesn’t mean you’ll suddenly remember every password, birthday and grocery item you’ve ever forgotten. The improvement seen in research is measured through specific cognitive tests and tends to be modest.

Still, memory is one area where creatine deserves more attention.

Does Everyone Get the Same Benefit?

Probably not.

Your diet, age, baseline creatine levels, sleep, health status and the type of cognitive stress you’re experiencing may all matter.

For example, people who consume little or no animal food may have lower dietary creatine intake. Some research suggests that certain groups could respond differently to supplementation.

The 2024 meta-analysis found stronger effects in some subgroups, including people with certain health conditions, adults aged 18 to 60 and females. However, subgroup findings shouldn’t be treated as proof that creatine will work better for every person in those categories.

This is one reason scientists are still studying the question instead of giving everyone the same answer.

How Much Creatine is Usually Used?

Creatine monohydrate is the form used in the cognitive studies included in the 2024 meta-analysis.

For general creatine supplementation, 3 to 5 grams per day is a commonly used maintenance amount. You don’t need to assume that a larger dose will give you better mental performance.

Some research looking specifically at the brain has used different doses, including much higher amounts. That doesn’t mean you should copy those study protocols yourself.

If you’re considering creatine specifically for brain fog rather than exercise, it’s sensible to speak with a doctor or qualified dietitian, particularly if you have an existing medical condition or take regular medicines.

Is Creatine Safe?

For most healthy adults, creatine has a strong safety record when used appropriately.

A large analysis of 685 clinical trials involving more than 12,800 people who received creatine found no meaningful increase in overall side effects compared with placebo. Reported gastrointestinal symptoms and muscle complaints were uncommon.

There’s also an important point about kidney tests.

Creatine can raise blood creatinine levels. Creatinine is commonly used as a marker when doctors assess kidney function, so this increase can sometimes look worrying on a blood report. Recent research suggests that the rise in creatinine doesn’t necessarily mean that creatine has damaged the kidneys.

However, that doesn’t mean everyone should take creatine without medical advice. If you have kidney disease, abnormal kidney tests or other health concerns, talk to your doctor before using it.

What Should You Do if You Have Brain Fog?

Before buying a supplement, look at the basics.

Are you getting enough sleep? Are you drinking enough fluids? Are you skipping meals? Have you been under unusual stress? Are you taking a medicine that could affect alertness? Has the brain fog appeared suddenly or lasted for weeks?

These questions matter because persistent brain fog can sometimes point to an underlying issue that deserves attention.

Creatine may support certain aspects of cognitive performance, but it shouldn’t become a shortcut for ignoring sleep, nutrition or an ongoing health problem.

So, Should You Take Creatine for Brain Fog?
