Telmistal-H 80mg/12.5mg Tablet


Overview

Introduction

Telmistal-H 80mg/12.5mg Tablet contains two medicines, both of which help to control high blood pressure. It is generally used when one medicine alone is not controlling the blood pressure effectively. Lowering blood pressure helps to reduce your risk of having a heart attack or a stroke in the future.

You can take Telmistal-H 80mg/12.5mg Tablet with or without food. The dose will depend on your condition and how you respond to the medicine. Try to take it at the same time each day. It’s important to keep taking this medicine until your doctor tells you not to. It contains a diuretic (water pill) and will make you urinate more, so it is best to avoid taking this medicine within four hours of going to bed. Use this medicine regularly even if you feel better to get the most from it. High blood pressure does not usually have symptoms and if you stop taking it your risk of heart attack or stroke may increase.

The most common side effect is dizziness. Other side effects include feeling tired, nausea, diarrhea, flu-like symptoms, shortness of breath and changes to your heartbeat. Frequent urination is not a side effect of this medicine. It is the purpose of this medicine as frequent urination helps control your blood pressure. You may experience other possible side effects, but most are rare. Make sure you read the leaflet that comes with your medicines and tell your doctor if you are bothered by side effects or they do not go away. It may be possible to adjust your dose or try a different medicine.

Do not use potassium supplements or salt substitutes unless your doctor has told you to. This medicine is not recommended during pregnancy or while breastfeeding. You should ask your doctor before taking it if you have kidney or liver disease, heart trouble or diabetes. Drinking alcohol can further lower your blood pressure and may cause side effects.

How to cope with side effects?

The occurrence of side effects varies from person to person. The following are a few ways of dealing with some of the common side effects. However, consult your doctor if these persist.

  • Coping with Dizziness
    This is usually short-lived and should go away within a few days. If this happens, stop what you are doing and sit or lie down until you feel better. Lying still in a dark, quiet room may help reduce the spinning feeling. Sleep with your head slightly raised on two or more pillows. Get up slowly from a lying or sitting position. Get plenty of rest and try to relax as being anxious can make it worse. Try taking this medicine at bedtime to reduce the symptoms. Drinking plenty of water and ginger tea may also help. Avoid alcohol, caffeine and smoking as it will make you feel worse. Avoid driving or using tools or machinery until you feel better.
  • Coping with Nausea
    You can help yourself by eating small, frequent meals rather than large ones and drinking plenty of fluids. Eat slowly. Avoid fatty, fried, spicy and very sweet foods. Eat cold or slightly warm food if the smell of cooked or cooking food makes you feel sick. Get plenty of fresh air. You could also try chewing ginger or drinking ginger tea. Eat bananas to replace potassium in your blood which can drop if you are sick (vomit). Use oral rehydration salts to replace vitamins and minerals lost through being sick. There are some medicines that can help you stop from feeling sick. Speak to your doctor if your condition does not improve.
  • Coping with Diarrhea
    Keep up your intake of fluids and electrolytes (sugars and salts) to avoid getting dehydrated. Eat less fiber (avoid raw fruits, fruit juice and vegetables). Talk to your doctor about possible medication to manage diarrhea. Ask about reducing the dosage of your drug or other suitable treatments.
  • Coping with Flu-like symptoms
    Get plenty of rest as exhausting or stressing out yourself could aggravate your condition. You should drink plenty of fluids as it will keep your throat moist and comfortable. Try taking steam inhalation. The moisture can ease your breathing and loosen mucus. You can consider sipping warm water or tea with honey and lemon. This will keep your nose and throat from being too dry. Using an air humidifier may also help. You can also gargle with salt water several times a day. See your doctor if your symptoms last longer than 3 weeks or get worse quickly. Avoid alcohol and do not smoke as they can aggravate your conditions.

How to use Telmistal-H Tablet

Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Telmistal-H 80mg/12.5mg Tablet may be taken with or without food, but it is better to take it at a fixed time.

How Telmistal-H Tablet works

Telmistal-H 80mg/12.5mg Tablet is a combination of two medicines: Telmisartan and hydrochlorothiazide which lowers the blood pressure effectively. Telmisartan is an angiotensin receptor blocker (ARB). It works by blocking the hormone angiotensin, thereby relaxing the blood vessels. This allows the blood to flow more smoothly and the heart to pump more efficiently. Hydrochlorothiazide is a diuretic that removes extra water and certain electrolytes from the body. Over time it also relaxes blood vessels and improves blood flow.

Telmistal-H 80mg/12.5mg Tablet is probably unsafe to use during breastfeeding. Limited human data suggests that the drug may pass into the breastmilk and harm the baby.
Driving

Telmistal-H 80mg/12.5mg Tablet may decrease alertness, affect your vision or make you feel sleepy and dizzy. Do not drive if these symptoms occur.
Kidney
CAUTION
Telmistal-H 80mg/12.5mg Tablet should be used with caution in patients with severe kidney disease. Dose adjustment of Telmistal-H 80mg/12.5mg Tablet may be needed. Please consult your doctor.
Use of Telmistal-H 80mg/12.5mg Tablet is not recommended in patients with severe kidney disease.
Liver
CAUTION
Telmistal-H 80mg/12.5mg Tablet should be used with caution in patients with liver disease. Dose adjustment of Telmistal-H 80mg/12.5mg Tablet may be needed. Please consult your doctor.

Q. What should I avoid while taking Telmistal-H?

You should avoid potassium supplements or salt substitutes, unless advised by your doctor. Also, avoid alcohol consumption as doing so can further lower your blood pressure and may cause some side effects. In addition to that, avoid getting dehydrated, drink enough fluids during exercise and in hot weather. The medicine may make you dizzy so avoid driving, using machinery or doing anything that needs concentration unless you know how this medicine affects you. If you feel dizzy, lie down so you do not faint. Then sit for a few moments before standing to prevent feeling dizzy again.

Q. What happens if I take too much of Telmistal-H?

If you accidentally take too much of Telmistal-H, you may experience symptoms such as low blood pressure, dizziness, lightheadedness, weakness, vomiting and rapid heartbeat. However, in some cases slow heartbeat, reduced kidney function including kidney failure (e.g., reduced urine output) have also been reported. If you experience any of such symptoms contact your doctor immediately and seek medical help.

Q. Can I come off Telmistal-H when my blood pressure is under control or normal?

Remember, Telmistal-H does not cure your hypertension (high blood pressure) but helps normalize it. You should continue taking this medicine even if your blood pressure is controlled. If you stop taking Telmistal-H, your blood pressure could rise back up again. Generally, blood pressure-lowering medicines are to be taken for the rest of your life. It is important to know that by keeping your blood pressure controlled you are reducing your risk of having a heart attack or a stroke.

Q. What happens if I forget to take Telmistal-H?

If you forget to take a dose, take it as soon as you remember. However, if it is close to the time of your next dose, skip the missed one and take the next scheduled dose. Do not take a double dose to make up for the one you missed. If you often forget doses, it may help to set an alarm to remind you. If not sure then you can consult your doctor for other ways to help you remember.

Q. What will happen if I stop taking Telmistal-H?

You should not stop taking Telmistal-H without talking to your doctor first. If you do stop taking it, your blood pressure may rise and, again, you will put yourself at greater risk of having a heart attack or a stroke. If any of the side effects such as dizziness, nausea, diarrhea etc., are bothering you, talk to your doctor.

Q. Is it safe to take Telmistal-H for a long time?

Telmistal-H is generally safe to take for a long time. Remember to follow your doctor's advice/instructions.

Q. How long will I be taking Telmistal-H for?

You may have to take Telmistal-H for a long-term or even lifelong. The medicine only keeps your blood pressure under control for as long as you are taking it. Therefore, it is advised to continue your medication and follow your doctor's advice.

Q. What if Telmistal-H doesn’t make me feel any better?

High blood pressure does not usually have any symptoms, so you may not feel ill when you are suffering from it. Therefore, this medicine may not make you feel “better”. However, it is working in the background to reduce your risk of serious illness such as heart attack and stroke.

Q. Can Telmistal-H affect my fertility or sex life?

Telmistal-H may very rarely lead to erectile dysfunction (inability to gain or maintain an erection). Consult your doctor if you are worried or notice some changes. However, untreated hypertension (high blood pressure) itself can lead to erectile dysfunction, therefore it is important that you take your medicine regularly and keep a track of your blood pressure.
