Introduction

Docel 20mg Injection is an anti-cancer medication used for the treatment of breast cancer, non-small cell lung cancer, prostate cancer and cancers of head and neck.

It is given as an injection by a qualified medical professional. Your doctor will decide what dose is necessary and how often you need to take it. This will depend on what you are being treated for and may change from time to time. You should take it exactly as your doctor has advised. Taking it in the wrong way or taking too much can cause very serious side effects. It may take several weeks or months for you to see or feel the benefits but do not stop taking it unless your doctor tells you to.

Infection and decrease blood cell count are some common side effects of this medicine. It may also cause serious side effects such as swelling of hands, lips, hair loss, and weight gain. To reduce these side effects your doctor may suggest some medication along with it. But, inform your doctor if you notice unexplained bruising or bleeding, sore throat, mouth ulcers, high temperature (fever). This medicine may reduce the number of blood cells (decrease red blood and white blood cells) in your blood, thereby, increasing the susceptibility to infections. Regular blood tests are required to check your blood cells and liver function. Your doctor may get regular blood tests done to monitor your blood cells and liver function during treatment with this medicine.

Before taking it, tell your doctor if you have heart disease, liver, or kidney problems or are taking any medicines to treat infections. Many other medicines can affect, or be affected by, this medicine so let your doctor know all medications you are using. This medicine is not recommended during pregnancy or while breastfeeding. The use of effective contraception by both males and females during treatment is important to avoid pregnancy.

How to cope with side effects?

The occurrence of side effects varies from person to person. The following are a few ways of dealing with some of the common side effects. However, consult your doctor if these persist.

  • Coping with Breathlessness
    Control your breaths by breathing in slowly through your nose and breathing out through your mouth. Sitting on a chair and leaning your chest slightly forward may also help. Increase the flow of air around you by opening windows or using a fan. Try to learn breathing techniques and do regular physical exercise. Wearing loose clothing around your waist and chest may also be helpful. Your breathing will usually return to normal within a few weeks of starting the medicine. Tell your doctor if your symptoms do not improve or get worse. Shortness of breath can also be a sign of an ongoing or worsening heart problem.
  • Coping with Constipation
    Try to eat foods which are high in fiber such as fresh fruits, vegetables and cereals to increase the bulk and consistency of bowel movements. Drink plenty of water or non-alcoholic drinks and stay hydrated to promote healthy digestion. Increasing physical activities such as walking, yoga and regular exercise can also be helpful. If your condition does not improve, you can ask your doctor to prescribe you laxative medicine.
  • Coping with Infection
    If you have a low level of white blood cells, you are more likely to get infections. There are several ways to reduce your chances of catching or spreading an infection. Avoid unnecessary exposure to germs when you can. Wash your hands frequently and thoroughly with soap or use a sanitiser. Avoid crowds and avoid contact with people who are ill or have infections. Use disposable tissues for coughs and sneezes. Consider using an appropriate face mask. Do not share personal items like cutlery, glasses, towels or toothbrushes with others. Get enough rest and eat a well balanced healthy diet.
  • Coping with Loss of appetite
    It is important to eat, even if you do not feel like it. You might try switching to smaller meals and frequent snacks. Try eating healthy and nutritious snacks that are high in calories and protein, such as dried fruit and nuts. It is important to drink even if you cannot eat. But, do not fill your stomach with liquid before eating. Eat when you are most hungry. Exercise such as a short walk may also increase your appetite. Talk to your doctor about possible medication or supplements if your condition does not improve.
  • Coping with Muscle pain
    Tell your doctor straight away because sometimes the muscle pain may be a sign of something more serious. A lower dosage or a different medication might be needed. Try to relax the cramping muscle by stretching it and holding the stretch. You might massage the muscle or apply a heat pad to the area after stretching. Avoid exercising too much. Stay well-hydrated. Do not take over-the-counter pain relievers unless the doctor prescribes it. Your doctor may also suggest taking supplements, such as calcium, magnesium oxide or potassium oxide depending on your needs.
  • Coping with Pain
    Some people use herbal remedies, acupuncture and psychological therapies to manage pain. Using hot and cold compress can also help reduce the pain temporarily. Ask your doctor about suitable painkillers if it bothers you or does not go away.
  • Coping with Weakness
    Make sure you rest and drink plenty of fluids. Eat a well-balanced diet to keep your energy levels up. Do not drink too much alcohol. Do not drive or use tools or machinery until you feel better. Other things that can help include doing some gentle exercise every day, prioritizing and pacing your activities and having a short nap if you need to. If you are still having problems after a week, speak to your doctor, as they may want to change you to a different type of medicine.

Q. Does Docel cause weight gain?

Yes, Docel may cause weight gain. It may occur due to fluid retention which may initially make your feet swell and then gradually it will spread to the whole body. Call your doctor immediately if you notice swelling of feet.

Q. How long do I have to take Docel?

You should continue to take Docel as long as your doctor advises you. The duration of treatment varies with the type of cancer you have and other factors like your response to treatment. You should usually receive your infusion once every 3 weeks.

Q. Can I drink alcohol while on Docel?

Docel injection contains alcohol because of which you may experience confusion, stumbling, becoming very sleepy, or feeling like you are drunk. This may impair your ability to drive or use machinery right after receiving it. Hence, you should avoid alcohol during Docel therapy, as it may affect you adversely.

Q. What is Docel used for?

Docel is commonly used for breast cancer, certain types of lung cancer, prostate, stomach, and head and neck cancers. It works by restricting cells from growing and multiplying.

Q. Is Docel available in pill form?

No, Docel injection comes as a liquid to be given intravenously (into a vein) by a doctor or nurse in a hospital or clinic. It is usually given over 1 hour once every 3 weeks.

Q. What blood tests are required before administration of Docel?

Your doctor will ask you to get blood tests every time you have a scheduled dose of Docel. You may be asked to have blood tests, such as hemogram and liver function tests, to ensure that you have enough blood cells and that your liver is functioning well enough to receive Docel. In case of white blood cells disturbances, you may experience associated fever or infections.

Q. Is hair loss caused by Docel irreversible?

Hair loss is a common side effect of Docel. Normal hair growth may resume after stopping Docel. However, in some cases there is permanent hair loss. Consult your doctor if you have any concerns regarding hair loss.

Q. Why is dexamethasone used with Docel?

Docel may cause fluid retention (swelling in the hands, feet, ankles and legs) as well as severe hypersensitivity reactions. To control such possibilities from occurring all patients are given corticosteroids (dexamethasone) for 3 days starting 1 day prior to Docel administration. In case of prostate cancer (metastatic castration-resistant), the recommended regimen is oral dexamethasone given at 12 hours, 3 hours, and 1 hour before the Docel infusion.
