Understanding Pancreatic Elastase

Pancreatic elastase is an enzyme produced by the pancreas to help digest food (proteins). Since elastase remains stable as it passes through the gastrointestinal tract, its levels in the stool directly reflect pancreatic output.

This test is especially useful for diagnosing exocrine pancreatic insufficiency (EPI), where the pancreas does not release enough digestive enzymes. EPI can occur due to conditions such as chronic pancreatitis, cystic fibrosis, diabetes, or pancreatic cancer. Individuals may experience symptoms like bloating, unexplained weight loss, diarrhea, and greasy or foul-smelling stools.

For this test, collect a fresh stool sample in a clean, sterile, leak-proof screw-capped container, preferably one with a spoon. Only a small portion of about 5–10 grams (1–2 teaspoons) is needed, so avoid overfilling the container. Do not use matchboxes, paper, or other absorbent materials for collection, and ensure the sample is not contaminated with urine or water.
