Creatinine is a chemical waste product formed due to the normal breakdown of muscles in the body. It is filtered by the kidneys and eliminated via the urine. Normally, creatinine is excreted out in urine, but the detection of abnormal levels (both high and low) of creatinine can be a sign of impaired filtration capacity of the kidneys. 

Your doctor may advise the Urine Creatinine, Random if you show symptoms suggestive of kidney disease, including frequent tiredness, dry and itchy skin, poor appetite, swelling in your feet or ankles, puffiness around your eyes, frequent urination, foamy and bloody urine, painful urination, muscle cramps, difficulty sleeping, etc. This test may also be suggested if you have risk factors for any kidney disease, such as diabetes, obesity, heart problems, or a family history of kidney disease. A lab technician will provide you with a sterile plastic cup with a lid for a urine sample. Collect the mid-stream sample in the provided container and carefully place the lid. Make sure that the container doesn't come in contact with your skin.
High creatinine levels are usually caused due to dehydration, increased protein intake, intense exercise, red meat consumption, or creatine supplementation, which are usually taken by athletes to enhance performance.
High creatinine levels might be an indication of acute or chronic kidney failure, urinary tract obstruction, hypothyroidism, nephrotoxic drugs, shock, and congestive heart failure. Always consult your doctor if your reports show high levels of creatinine.
Low levels of urinary creatinine might indicate a problem with the muscles or liver but may also be due to something less serious, such as reduced muscle mass in older adults, drinking too much water (psychogenic polydipsia), or pregnancy.
