Understanding PML-RARA, Quantitative

The PML-RARA, Quantitative test is a molecular test that looks for and measures the PML-RARA fusion gene or its RNA copies in blood or bone marrow. In APL, parts of chromosome 15 and chromosome 17 break and swap places, creating this fusion gene. The abnormal protein made by this gene prevents white blood cells from maturing properly, causing a buildup of immature cells called promyelocytes, which leads to leukemia.

 

Normally, the PML gene stops uncontrolled cell growth, and the RARA gene helps white blood cells develop properly. When fused, they lose these functions, leading to abnormal cell growth. Around 98% of APL cases have this t(15;17) PML-RARA fusion, while rare cases may have other fusion genes.

 

Detecting PML-RARA is crucial to confirm APL and to guide treatment. Standard therapy includes all-trans retinoic acid (ATRA), often with chemotherapy. ATRA works specifically on cells with the PML-RARA gene, helping them mature and improving treatment outcomes. Measuring PML-RARA levels also helps doctors detect remaining leukemia cells after treatment and assess the risk of relapse.

 

Doctors may recommend this test if a patient has symptoms like easy bruising, bleeding, infections, abnormal blood counts, or if a blood disorder is suspected. It is also used to monitor patients undergoing APL treatment.
