Introduction of Tamsulosin

Toltam Capsule is a combination of two medicines. It is used in the treatment of benign prostatic hyperplasia. It helps to relax the muscles of the bladder and prostate gland thereby relieving pain and irritation. It also helps to control the urge to urinate.

Toltam Capsule is advised to take it in a dose and duration as per prescription. It should be taken with food, but take at the same time regularly. Swallow the medicine as a whole without crushing or chewing it. You should not stop taking the medicine without consulting the doctor as it may lead to the worsening of your symptoms. The course of the treatment should be completed for better efficacy of the medicine.
Use of this medicine can cause some common side effects such as dizziness, ejaculation disorder, and retrograde ejaculation. If any of the side effects persist or bother you, you must consult the doctor without delay.
Simple lifestyle changes can help you manage your symptoms better. Try to urinate as soon as you feel the urge, however, never strain or push to empty your bladder. You should avoid drinking caffeinated or alcoholic drinks at night and a few hours before going to bed or going out.
Before receiving the treatment, inform your doctor if you are on any medication for any other health condition. If you have to undergo eye surgery due to cataracts or glaucoma then inform your eye doctor about the usage of this medicine. Patients with liver disease must be cautious while receiving the prescription, and they must receive regular follow-ups as per the doctor’s advice.

What if you forget to take Toltam Capsule?

Q. What is the best time to take Tamsulosin?

You can take the capsule after breakfast or after the first meal of the day. However, some doctors prefer it to be taken at night time after dinner. You should take it approximately 30 minutes following the same meal every day. Swallow it as a whole with a glass of water. Do not open, crush or chew the medicine.

Q. Can the use of Tamsulosin increase blood sugar levels?

Use of Tamsulosin does not increase your blood sugar levels. If you notice any change in the blood sugar levels while taking it, consult your doctor as this could be due to some condition which needs attention.

Q. Does Tamsulosin cause weight gain?

No, Tamsulosin does not usually cause any increase in the weight, even on long-term use. If you experience any weight gain while taking the medication, then consult your doctor to identify the cause of the weight gain.

Q. Why does Tamsulosin cause dizziness?

Tamsulosin dilates the blood vessels and leads to the accumulation of blood in the extremities (ends of the arms and limbs). This restrains the proper flow of blood to the brain, thereby causing the blood pressure to fall on the sudden change in posture. As a result, the patient may experience dizziness, lightheadedness, fainting, spinning sensation, and vertigo.

Q. Why should I stop taking Tamsulosin before my cataract surgery?

Tamsulosin can cause floppy eye syndrome in which the muscles of the iris become floppy and the pupil constricts unexpectedly during cataract surgery. So, when the eye surgeon actually needs a dilated pupil, it will constrict and restrict the field of surgery, and can affect the surgical outcomes.

Q. Why does Tamsulosin cause stuffy nose?

Tamsulosin relaxes the smooth muscles of the blood vessels and this vasodilation of the blood vessels can cause a stuffy nose.

Q. Is Tamsulosin anticholinergic?

No, Tamsulosin is not an anticholinergic medication. It is a medicine that acts by relaxing the muscles in the prostate gland and neck of the urinary bladder, improving the urine flow in patients with benign enlargement of the prostate gland. Taking anticholinergic drugs in this condition or while taking Tamsulosin can increase the symptoms and worsen the condition.

Q. Can Tamsulosin cause frequent urination?

No, Tamsulosin does not cause frequent urination. In fact, this medicine increases the urine flow and decreases the urge to urinate frequently, which is a symptom of benign enlargement of the prostate gland. This condition also leads to a weak stream while urinating and hence the patient is not able to empty the bladder completely. Tamsulosin is used to treat all such symptoms related to urine flow in patients with benign enlargement of the prostate gland.

Q. What are the long-term effects of taking Tamsulosin?

Usually Tamsulosin does not cause any side effects in patients with benign enlargement of the prostate gland. However, long-term use of Tamsulosin may increase the chances of having infection, rhinitis, pain, and pharyngitis. Its long-term use may also cause abnormal ejaculation, blackouts, faintness, lightheadedness, dizziness, and decreased blood pressure.

Q. Apart from taking medicines, what should I do to manage my prostate symptoms?

Simple lifestyle changes can help you manage your symptoms better. Try to urinate when you first get the urge. However, take care not to strain or push to empty your bladder. Avoid drinking fluids (especially alcohol, caffeine) a few hours before bedtime or going out. Also, you should not take any other medicine without consulting your doctor because some medicines can increase your urge to urinate.

Q. Can I take Tamsulosin with ibuprofen or paracetamol?

Tamsulosin can be taken with ibuprofen or paracetamol. No harmful side effects or other interactions have been seen when they are taken together.
Tolterodine

Q. How long does it take for Tolterodine to show its effect?

Your symptoms may start improving within 1 week of starting Tolterodine. Maximum benefits may be seen after 5-8 weeks of treatment. To maintain this improvement, your doctor may prescribe this medicine to you for a long term of up to 24 months.

Q. What class of drug is Tolterodine? Is it a diuretic?

Tolterodine belongs to a class of medicines known as muscarinic receptor blockers. Tolterodine is not a diuretic, it is a urinary antispasmodic. This means that it relaxes the urinary bladder, decreasing spasm of the bladder wall. This further provides better control upon the release of urine and also increases storage volume of the bladder.

Q. Can I just stop taking Tolterodine?

Tolterodine does not cure your condition but helps to control the symptoms of overactive bladder. Do not stop taking this medicine even if you feel better. Your doctor will reassess your condition at regular intervals, like 6 months, to understand the duration for which you need to take Tolterodine.

Q. Can I crush the tablets of Tolterodine and then take it?

No, do not crush or chew the tablets. Swallow the whole tablet with a glass of water. You can take it with or without food. Take it exactly as directed by your doctor.

Q. Can Tolterodine be used in children?

When studied in children, Tolterodine did not relieve the symptoms of overactive bladder. Therefore, its use in children is not recommended.

Q. Who should avoid taking Tolterodine?

Patients who have urinary retention (unable to pass urine), have delayed emptying of stomach or blocked intestinal tract (toxic megacolon, ulcerative colitis, pyloric stenosis) or uncontrolled high pressure in the eyes (narrow-angle glaucoma). It should also be avoided by patients who have any muscle disease which causes drooping eyelids, double vision, difficulty in speaking and swallowing, and sometimes muscle weakness in the arms or legs (myasthenia gravis). Patients who are hypersensitive to Tolterodine should avoid it as well.

Q. I am having constipation because of which I want to discontinue Tolterodine. Will I have any harmful effects?

No, discontinuing Tolterodine will not have any harmful effects, but the symptoms of overactive bladder may return. Talk to your doctor regarding constipation and you can stop the medicine if advised by the doctor.

Q. Does Tolterodine cause dementia?

It is unclear whether Tolterodine can cause dementia or not. However, according to some reports, when people with dementia were given Tolterodine, their symptoms (confusion, disorientation and delusion) became worse.
