Understanding tTG (Tissue Transglutaminase) - IgG antibody

Tissue transglutaminase is an enzyme in the body that is involved in normal protein processes. In people with celiac disease, the immune system mistakenly creates antibodies against this enzyme. These are called anti-tissue transglutaminase antibodies and are used to help diagnose celiac disease. 

While IgA anti-tTG is the usual first test, the tTG (Tissue Transglutaminase) - IgG antibody is mainly helpful for patients with IgA deficiency to avoid false-negative results.

Doctors may suggest the tTG (Tissue Transglutaminase) - IgG antibody test when there are symptoms of celiac disease, such as stomach pain, bloating, chronic diarrhea, weight loss, failure to thrive (in children), anemia, dermatitis herpetiformis (an itchy, blistery rash), etc. This test is particularly used as an alternative for people who have an IgA deficiency, as they may not get accurate results. It can also be done to monitor the effectiveness of a gluten-free diet. Additionally, it is useful for screening individuals at higher risk, such as those with a family history of celiac disease or other autoimmune conditions. 

No special preparation is required before undertaking the tTG (Tissue Transglutaminase) - IgG antibody test. For accurate diagnosis, continue to eat foods that contain gluten for a time period, such as several weeks, before testing only if advised by the doctor. The adult patient eats high amounts of gluten (usually 8-10 gm of gluten per day, equivalent to approximately 4-6 slices of bread a day) for 6-8 weeks. This gluten dose can be gradually built up over the first week. A repeat celiac screening test is done after the gluten challenge. This type of challenge can be poorly tolerated by patients, and thus, some may be resistant to this approach.
