Chronic myeloproliferative neoplasms (MPNs) are a group of blood disorders where the bone marrow produces too many red blood cells, white blood cells, or platelets. This uncontrolled cell growth can lead to complications such as blood clots, bleeding problems, or progression to acute leukemia.

The Myeloproliferative Neoplasm (MPN) Reflex Panel With BCR-ABL combines next-generation sequencing (NGS) with reflex testing to identify mutations in the BCR-ABL1, JAK2, MPL, and CALR genes. All of these genes play a vital role in the diagnosis and classification of MPNs.

This test works in a stepwise or “reflex” manner. It first screens for the BCR-ABL1 fusion gene, the hallmark of chronic myeloid leukemia (CML). If BCR-ABL1 is negative, the test reflexively analyzes other MPN-related genes known as JAK2 (including the common V617F and exon 12–15 mutations), followed by MPL and CALR if needed.

By examining these genetic regions sequentially, the test distinguishes CML (BCR-ABL–positive) from BCR-ABL–negative MPNs, such as polycythemia vera (PV), essential thrombocythemia (ET), and primary myelofibrosis (PMF).
