The Myeloproliferative Neoplasm (MPN) Reflex Panel With BCR-ABL is a comprehensive genetic test that identifies key mutations linked to blood cancers known as myeloproliferative neoplasms (MPNs). It helps detect and differentiate chronic myeloid leukemia (CML) from other MPNs, guiding accurate diagnosis, treatment decisions, and disease monitoring.

Understanding Myeloproliferative Neoplasm (MPN) Reflex Panel With BCR-ABL

Chronic myeloproliferative neoplasms (MPNs) are a group of blood disorders where the bone marrow produces too many red blood cells, white blood cells, or platelets. This uncontrolled cell growth can lead to complications such as blood clots, bleeding problems, or progression to acute leukemia.

The Myeloproliferative Neoplasm (MPN) Reflex Panel With BCR-ABL combines next-generation sequencing (NGS) with reflex testing to identify mutations in the BCR-ABL1, JAK2, MPL, and CALR genes. All of these genes play a vital role in the diagnosis and classification of MPNs.

This test works in a stepwise or “reflex” manner. It first screens for the BCR-ABL1 fusion gene, the hallmark of chronic myeloid leukemia (CML). If BCR-ABL1 is negative, the test reflexively analyzes other MPN-related genes known as JAK2 (including the common V617F and exon 12–15 mutations), followed by MPL and CALR if needed.

By examining these genetic regions sequentially, the test distinguishes CML (BCR-ABL–positive) from BCR-ABL–negative MPNs, such as polycythemia vera (PV), essential thrombocythemia (ET), and primary myelofibrosis (PMF). This comprehensive approach ensures an accurate molecular diagnosis, supports personalized treatment planning, and helps doctors monitor disease progression and treatment response effectively.

Doctors may recommend this panel if you experience symptoms such as fatigue, weakness, night sweats, or an enlarged spleen, or if you have abnormal blood counts. It is also advised for individuals with a family history of MPNs or known genetic risk factors. This test helps diagnose specific MPNs, differentiate between subtypes for appropriate treatment, monitor therapy response, and assess complication risks.

For this test, it is important to submit the clinical history, histopathology report, previous test results, and a doctor’s prescription at the time of sample collection. The Tata 1mg consolidated Test Requisition Form (TRF) must also be duly filled and submitted.

Lab test results may vary across different laboratories. Abnormal test results require an expert interpretation. Therefore, never try to self-medicate at home based solely on these results, and always consult a doctor for proper understanding of the test results.

What does Myeloproliferative Neoplasm (MPN) Reflex Panel With BCR-ABL measure?

The Myeloproliferative Neoplasm (MPN) Reflex Panel With BCR-ABL measures specific genetic mutations that drive abnormal blood cell production. It detects the BCR-ABL1 fusion gene, which confirms chronic myeloid leukemia (CML), and reflexively analyzes JAK2, CALR, and MPL mutations to identify BCR-ABL–negative MPNs such as polycythemia vera (PV), essential thrombocythemia (ET), and primary myelofibrosis (PMF).

By combining these analyses, the test provides a complete molecular picture of the disease, helping doctors confirm the exact MPN subtype, predict disease behavior, and plan safe and effective treatment strategies. Abnormal results indicate the presence of genetic mutations responsible for uncontrolled blood cell production, guiding doctors in making precise and personalized therapeutic decisions.

FAQs related to Myeloproliferative Neoplasm (MPN) Reflex Panel With BCR-ABL

This is a genetic test that detects mutations in the JAK2, MPL, and CALR genes, as well as the BCR-ABL fusion gene, to diagnose and classify myeloproliferative neoplasms (MPNs) including chronic myeloid leukemia (CML), polycythemia vera (PV), essential thrombocythemia (ET), and primary myelofibrosis (PMF).
The test works in a stepwise manner: it first checks for the JAK2 V617F mutation. If negative, it reflexively tests other regions of JAK2, then MPL, then CALR, and finally detects BCR-ABL using RT-PCR. This ensures a thorough and efficient evaluation.
It is recommended for individuals with persistently high blood counts, symptoms such as fatigue, night sweats, or enlarged spleen, or those needing differentiation between BCR-ABL–positive CML and other MPNs. It is also advised for monitoring disease progression and treatment response.
A phlebotomist will clean your skin using an antiseptic alcohol cotton swab or wipe and take blood samples from your vein using a needle. The blood sample will be stored safely and transported to the laboratory for analysis.
No fasting is required for this test. You can schedule it at your convenience without avoiding food or water beforehand.
No, the Myeloproliferative Neoplasm (MPN) Reflex Panel With BCR-ABL is generally safe with minimal risks. However, you may feel slight discomfort during blood sample collection but chances of serious complications are extremely rare.
Abnormal results indicate the presence of specific gene mutations or the BCR-ABL fusion gene. These findings help doctors accurately classify MPNs and guide safe and effective treatment planning.
BCR-ABL-positive MPNs, like chronic myeloid leukemia, are driven by the BCR-ABL fusion gene. BCR-ABL-negative MPNs are caused by other gene mutations, such as JAK2, CALR, or MPL. This panel detects both types.
Unlike routine blood tests, this panel uses advanced molecular techniques such as NGS and RT-PCR to detect specific gene mutations and fusions that are not visible in standard blood work.
