The Metanephrine-Free, Plasma test checks for rare tumors called pheochromocytomas and paragangliomas. These tumors can cause the adrenal glands to release too much of the body's stress hormones: epinephrine (adrenaline) and norepinephrine (noradrenaline). When these hormones break down, they form substances called metanephrines. This test measures the level of free metanephrines in the blood.

Doctors may suggest this test if signs of excess adrenaline-like hormone are present, or if a tumor like pheochromocytoma is suspected based on symptoms or scan results. It is also used to monitor individuals who have had these tumors in the past. This test can be done if there are symptoms like high blood pressure that comes and goes, a fast heartbeat, headaches, and sweating. 

An overnight fast for 8–12 hours is required before the test. However, drinking water is acceptable. Avoid taking specific medicines like labetalol, sotalol, acetaminophen, methyldopa, antidepressants, and agents such as ephedrine, amphetamine, nicotine, and caffeine before the test. Foods like bananas, pineapples, nuts, and cereals should also be avoided for at least 72 hours before sample collection.
