The Maternal Screen (QUAD test) - 1st Trimester with Preeclampsia test is a set of blood tests done early in pregnancy, usually between 11 and 13 weeks + 6 days. This maternal screening test evaluates the risk of certain genetic conditions in the baby like Down syndrome, Edward's syndrome & Patau's syndrome, as well as maternal complications like preeclampsia (PE), which is a condition of high blood pressure in pregnant women.

Understanding Maternal Screen (QUAD test) - 1st Trimester with Preeclampsia

This screening measures the levels of several important substances in the mother’s blood, including:

  • Free beta-human chorionic gonadotropin (free beta-hCG)
  • Pregnancy-associated plasma protein A (PAPP-A)
  • Alpha-fetoprotein (AFP)
  • Placental growth factor (PLGF)

The Maternal Screen (QUAD test) - 1st Trimester with Preeclampsia test checks for these blood markers in the first trimester, along with ultrasound findings and details like blood pressure of the pregnant female. Based on an algorithm, the results are expressed as MoM (Multiples of the Median) values. Using MoM values helps make the test more reliable by maximizing the detection rate and minimizing false positives. This test helps assess risk of fetal chromosomal abnormalities as well as early detection of preeclampsia risk. Early detection can help support prenatal counseling and decision-making and managing high-risk pregnancies.

Abnormal levels in these markers may indicate a higher chance of certain chromosomal conditions, such as Down syndrome (Trisomy 21), Edwards syndrome (Trisomy 18), Patau syndrome (Trisomy 13), or neural tube defects. Nevertheless, this test only estimates the risk; an increased risk result does not mean that the fetus is affected, and a low-risk result does not mean that the fetus is unaffected. For confirmation, the reported risks should be correlated according to the absence/presence of sonographic markers observed in the anomaly/malformation scan. 

Preeclampsia affects 2%–5% of pregnant women and can be serious for both mom and baby, especially if it starts early. Because of this, the International Federation of Gynecology and Obstetrics (FIGO) in 2019, recommended that all pregnant women get screened for preeclampsia in early pregnancy. Identifying women at high risk helps doctors give the right care in time, and, in some cases, preventive treatment with low-dose aspirin may be suggested.
