Macro-TSH refers to the presence of large protein-bound complexes of TSH in the blood, which are not biologically active. These complexes can lead to incorrectly high TSH levels when measured using standard TSH assays. This test is used in the evaluation of subclinical hypothyroidism, a condition characterized by mildly elevated thyroid-stimulating hormone (TSH) levels and normal levels of thyroid hormones, specifically free thyroxine (FT4). This is done by measuring the proportion of free TSH and the percentage of recovery after precipitation of the macro-TSH complexes.
