Overview of Widal Test (Slide Agglutination)

Test Description

Typhoid fever also called enteric fever is caused by bacterium Salmonella Typhi and is usually transmitted through foods and drinks contaminated with faecal matter.You should get this test done if you have symptoms which suggest typhoid fever like fever, abdominal pain, headaches, rose spots. It helps to make presumptive diagnosis of typhoid fever.

Why Get Tested

  1. If you have signs or symptoms of typhoid (enteric) fever like headache, high fever, abdominal pain, diarrhea, weight loss or skin rashes called rose spots 
  2. As a part of fever panel

What Results of Widal Test (Slide Agglutination) mean?

Interpretations

  • If the TO antigen titre is greater than 1:160 in an active infection, the widal test is positive and indicates typhoid or enteric fever 
  • If the TH antigen titre is greater than 1:160 in past infection or immunized persons, the widal test is positive and indicates typhoid fever
  • If these antigens are negative, then the widal test is considered to be negative and there is no typhoid fever

  • A single Widal test holds little clinical applicability due to the several cross reacting infections, which include malaria. If other tests, such as serology or bacteriologic culture, are unavailable, then inferences such as a fourfold rise in the titre {For example from 1:30 to 1:120} during the course of an infection, or a change from an IgM reaction of a specific titre to an IgG reaction of the same titre in the least, would signify typhoid reaction
