S-Adenosyl-L-Methionine (SAMe), a naturally occurring compound in Ademetionine / S-Adenosyl Methionine can be used for alcoholic liver disease. It performs many functions in the liver, including major physiologic defense mechanism against oxidative stress. SAMe is particularly important in opposing the toxicity of free oxygen radicals generated by various pathogens and also alcohol which causes oxidative stress. Metformin is seen to be beneficial in the treatment of non-alcoholic fatty liver disease (NAFLD). One of the underlying cause of NAFLD is insulin resistance. Metformin improves insulin resistance and liver enzymes in these patients. It is not seen to benefit patients of alcoholic fatty liver disease and is usually avoided in patients with liver cirrhosis as they are at an increased risk of lactic acidosis.

Yes, fatty deposits in the liver may be a risk factor for development of cirrhosis. Excess accumulation of fat may cause inflammation of the liver and subsequent hardening and scarring giving rise to cirrhosis. Hence, overweight people are at increased risk of developing liver cirrhosis.

Pentoxifylline works by decreasing the level of TNF-α (Tumor necrosis factor) in alcoholic hepatitis. TNFα is a cell signaling protein (cytokine) involved in systemic inflammation and is increased in alcoholic liver disease (ALD), and alcoholic hepatitis (AH) in particular. Talk to your doctor regarding its use in hepatitis.
