  • All the BPL and APL (up to INR 5 lakhs per annum income) and resident of Assam state are eligible for the insurance scheme
  • There are about 436 approved procedures which are reimbursed under the scheme
  • How?

  • Village level and district level kiosks carry out the enrollment process after verifying the documents of the applicant and provide smart card upon enrollment
  • Location and schedule of the enrollment at village level in mobile kiosk will be carried out by the implementing agency
  • Enrollment will be free for BPL families, and for families with annual income of INR 1.5 lakhs to 5 lakhs, enrollment fees of INR 100/- per member will be charged annually
  • Where?

  • In all the government hospitals and empanelled private hospitals
  • Health coodinator (Arogya mitra) at every empanelled hospital will help beneficiaries
  • For enrollment into the insurance scheme, photo identity proof and voter's identity card are mandatory
  • For children under 18 years of age birth certificate is mandatory
  • For APL family (INR 1.5 lakhs to 5 lakhs per annum income), income certificate is required
  • Biomteric details, like fingerprints, is mandatory for each member of the family
  • Cashless benefit of INR 2 lakhs per annum per member of the family
  • Transportation allowance of INR 300/- per visit, subject to maximum of INR 3000 per annum
  • For treatment out of the state, daily allowance of INR 1,000/-, subject to maximum INR 10,000/-
  • For treatment out of the state in an empanelled hospital in selected city, economy cheapest airfare will be provided to the patient and one attendent
